Grommets (ear tubes)

Who this is for

Mostly children with glue ear (fluid behind the eardrum) that has lasted several months and affects hearing, speech or learning, or with frequent middle ear infections. Adults occasionally need grommets too. Many cases of glue ear settle on their own, so a period of watchful waiting is common.

How we assess

The surgeon examines the ears and reviews a hearing test and tympanogram (a test of how the eardrum moves). Adenoids may also be assessed, as large adenoids can contribute to glue ear.

The procedure

Under a short general anaesthetic (for children), a tiny opening is made in the eardrum, the fluid is cleared and a small tube is placed to let air into the middle ear. No external cuts are made. Adenoids are sometimes removed at the same time.

Risks

  • Discharge from the ear, which is usually treated with ear drops

  • The grommet coming out early or, less often, staying in too long and needing removal

  • A small hole remaining in the eardrum after the grommet comes out, which occasionally needs repair

  • Scarring of the eardrum (usually without effect on hearing)

  • Glue ear returning and further grommets being needed

  • Risks of general anaesthetic

Recovery (typical)

Most children go home the same day and return to normal activities within a day or two. Your surgeon will advise on water precautions. Grommets usually fall out on their own within about 6 to 18 months. Follow-up appointments check the ears and hearing.
